In 1928, Dr. Richter described a patient with chronic lymphocytic leukemia (CLL), who progressed to develop an aggressive large cell lymphoma. Richter syndrome (RS), also called Richter transformation, is now understood to describe the development of an aggressive lymphoma in patients with CLL or small lymphocytic lymphoma (SLL).
Though it is still very early on, BTK degrader BGB-16673 appears tolerable. Clinical trials of BTK degraders may be especially of interest to CLL patients
MRD guided acalabrutinib, venetoclax and obinutuzumab led to an 88% progression free survival rate in relapsed / refractory chronic lymphocytic leukemia.
A new phase 3 clinical trial is testing sonrotoclax, a new BCL2 inhibitor, in combination with zanubrutinib for CLL.
Chimeric antigen receptor T-cell (CAR-T) therapy can provide durable remissions in a subset of patients with Richter’s transformation.
